We
will:
♦ Identify
problems
We
provide a complete assessment of the medical/physical,
social and financial needs of our clients. This assessment
generates our care plan which lists specific goals, actions
to be taken and the time frame for completion. All care
plans are updated to continue to reflect the needs of the
individual.
♦ Evaluate
home safety
Included
in our assessment is a complete evaluation of the safety
of the home and ways that it can be improved upon including:
· Inspection of kitchen to ensure proper
nutrition.
· Decreasing hazards in bathrooms, walkways
and stairs.
♦ Arrange
for needed services
When
a specific service is required, we will research the available
possibilities and provide three choices so that the client
can make an informed decision. All services that are presented
will be from reputable companies providing affordable services
that meet the actual needs of the client.
♦ Respond
to crises immediately
When
a crisis situation arises, we will be available to be there
and provide any help or information that an emergency response
team will need. We will accompany the client to the hospital
and remain with them until they are admitted or the crisis
has stabilized
A
hurricane plan is part of our care plan and all steps will
be followed in the event that a hurricane warning is issued.
♦ Assist
in moving to new residence
If placement
in a facility or new residence becomes necessary, we will
again provide at least three choices that meets the client’s
needs and will continue to monitor their stay in any facility
to ensure that their rights are never infringed upon. We
will also assist the client in moving and ensure that the
necessary items are taken with them.
♦ Communicate
with family members
One
of our most important aspects of service is the ability
to relay accurate information on the health and well being
of our clients to family members who live out of the area.
We will be available to speak to family members at any
time. We will keep you informed in any emergency and provide
continual updates on any situation allowing you to have
the peace of mind that your loved one is always being looked
out for.

We
are on call 24 hours a day 7 days a week. We are always just
a phone call away.
