Gardens Professional Care Management, Inc. is a geriatric care management company located in South Florida serving Palm Beach and Martin Counties.

As care managers we will:

• Assist older adults in maintaining their independence.
• Work together with doctors, trust officers and other professionals.
• Screen, arrange for and coordinate all services that are required.
• Provide a needed link between our clients and family members who live out of the area.
• Advocate for the health, safety, security, quality of life and rights of older adults.
